Frequently asked questions
No, you do not need a doctor's referral to see a Physiotherapist. The only time you need a referral is if you want to claim the cost of your treatment through Workcover (i.e. you injured yourself at work), Veteran's Affairs, Medicare (you need an Enhanced Primary Care Plan) or 3rd Party Insurance (i.e. a car accident insurance claim).
For your convenience we provide the HICAPS system for on-the-spot processing of private health fund rebates (if you are covered for physiotherapy by your health fund you just swipe your health fund card, your rebate is processed automatically and you only have to pay the difference). Payment is required at the time of treatment and we accept cash, MasterCard, Visa and EFTPOS. If your treatment costs are being claimed through Workcover, Third Party or Veterans Affairs then once the appropriate paperwork is in place there is no cost to you. If you are claiming through Medicare EPC, you pay the Medicare fee in full and we we process your Medicare rebate and the difference will be reimbursed directly back to you within 48hrs
Cancellation & Non-Attendance Policy
Our goal is to provide quality health care to all our patients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our practitioners but our other patients as well.
Therefore, to promote continuity of care and to maximise available appointments for patients who need urgent treatment, we ask for 24 hours' notice if you are unable to attend your appointment.
How to Cancel Your Appointment If you are unable to attend the scheduled appointment time for any reason, please call us on 8068 6776 and inform our admin team as soon as possible. If you are unable to speak directly with us, please leave a voicemail message. We will return your call as soon as possible.
Late Notice If you provide us with less than 12 hours' notice, a Late Notice Fee of $50 will be charged.
Non-Attendance If you simply do not attend your appointment and have not provided us any notice, you will be charged the full fee of the appointment that was missed.
Compassionate exceptions may apply, however, repeat events will require full payment of your appointment before attending.
Comfortable clothing is recommended for your treatment sessions and we also provide gowns & towels whenever needed. This allows access to your injured area for effective assessment and treatment. Your modesty and privacy are extremely important to us and are fully protected at all times.
Here at Fine Form Physiotherapy we have 2 treatment rooms on the ground floor and 3 treatment rooms on the first floor to provide excellent provisions for the injured or less mobile clients. If you are too immobile to attend the clinic please consider our home visit service.
We have onsite parking with plenty of spots available and for street parking please park on Hillcrest Avenue.
Of course this is ok, we welcome the support of family members as this aids in the healing process. We have plenty of chairs in the treatment room for you to be able to sit comfortably whilst your relative is being treated.
